The Chestnut Adult Family Home, located in Port Washington, WI,  is classified as an Adult Family Home and is licensed for a maximum number of three residents.  The home is a duplex with the state licensed AFH on the lower level and a supported apartment on the upper level.

The Chestnut Adult Family Home is a state licensed facility that supports adults with Developmental Disabilities who are ambulatory.  This facility can support individuals who may exhibit significant behavioral challenges, can follow simple directions, and may require assistance for self-preservation under emergency conditions.

With the intimate environment, our employees can provide services and care to meet the clients’ individualized needs.

The assessment process consists of family interviews, review of background information, resident skill potential and orientation. Prospective residents have the option to visit any for our group homes for meals, activities, and possibly extended stays. These visits will help to ensure a smooth transition to their new home.
