Chestnut Adult Family Home

The Chestnut Adult Family Home is licensed by the state of Wisconsin's Department of Health and Family Services as a Community Based Residential Facility.  The home is licensed to accommodate three adults with developmental disabilities and can support individuals who are semi-ambulatory and need assistance evacuating in emergency's. This home is a state licensed duplex, an Adult Family Home on the lower level and a supported apartment on the upper level.

The residents at Chestnut Adult Family Home require more indivualized, one-on-one guidance from our staff. The staff to resident ratio is approximately 1:2. This allows staff to spend quality time with our residents in an effort to facilitate increased community integration. 

The staff at Chestnut Adult Family Home have all of the required training, in addition to twelve hours of continued educational training. The additional training for the staff is tailored to the needs of the residents. The staff at Chestnut Adult Family Home provide assistance with personal cares, activities of daily living, medication administration and much more.
